Job Description:

Davis Behavioral Health, Inc. is seeking a full-time employee to perform duties as an Clubhouse Generalist (Case Manager/Social Service Worker) at Journey House, a program which promotes the Clubhouse Model of Psychiatric Rehabilitation to members with severe and prolonged mental illness. This position is based at the clubhouse located at 2250 North 1700 West in Layton, Utah. The successful candidate will work under the Journey House Program Director and join a strong team of dedicated professionals.

Primary Job Duties

Duties are performed side by side with members. Coordinates and plans vocational, social and independent living skills, building activities associated with each unit by:

a. Implementing the Clubhouse Model of Psychiatric Rehabilitation with members ensuring skill development opportunities for each member according to Clubhouse Standards.

Management of Clubhouse

  1. Planning, coordinating, and executing daily unit work with membership
  2. Working side by side with membership to complete unit work and tasks
  3. Applying and modeling the Clubhouse Standards, as stated through the ICCD (International Center for Clubhouse Development), in daily tasks
  4. Creatively encouraging and motivating members to participate in their rehabilitation
  5. Organizing, executing, and participating in some Evening, Weekend, and Holiday activities
  6. Actively participating in the fundraising efforts of the Clubhouse
  7. Completing frequent outreach of members who are not in attendance for each unit
  8. Assisting to meet the community needs of members

b. Ensuring compliance with regulations that govern: Clubhouse International Certification, DHHS licensing, and State Medicaid regulations.

Documentation Requirements

  1. Developing Personal & Individualized Treatment Plans with members
  2. Completing reviews of the treatment plans with members
  3. Completing bi-weekly comments on progress notes with member in unit
  4. Completing documentation in adherence to DBH standards.

c. Providing supportive life coaching to members.

d. Providing coverage in all units as needed.

e. Providing on-site job coaching to members as needed to maintain successful Transitional Employment Placements (T.E.P.), Supported Employment, and Competitive Employment in the community.

  1. Participate in Transitional Employment as placement manager

f. Participate in 2 week clubhouse training.

g. Adhere to Clubhouse International Standards and Philosophy

h. Other responsibilities as assigned by Program Director or Supervisor.

Schedule

This is a full-time position and is normally assigned to weekdays. However, DBH-JH reserves the right to require overtime, weekend work, and/or assignment to evening or night shifts, depending upon operational need, some in and out of state travel may be required. All DBH employees may be re-assigned to another program depending on Center needs.

EDUCATION/CERTIFICATION

Bachelor degree in Social Work or related field of Human Services. Relevant experience in the mental health field.

EXPERIENCE REQUIRED

Prefer Clubhouse Model of Psychiatric Rehabilitation experience and/or willingness to work as Generalist Practitioner.

SKILLS/ABILITIES

Must be able to exercise judgment, work independently and effectively with a team under general direction.

PHYSICAL DEMANDS

The physical demands described here are representative of those that must be met by employee to successfully perform the essential functions of this job.

The employee must be able to drive and must have a valid driver’s license for six years minimum with clean driving record to be covered by DBH insurance. The employee is required to stand, walk, reach, carry, and kneel. The employee must be able to walk on uneven surfaces and climb stairs. The employee must occasionally move or lift 25-50 pounds. The employee must be able to sit at computer workstations for up to 3 hours a day. The employee must be able to perform multiple job tasks at places of Transitional Employment Sites, equivalent to the physical demands of working at the Clubhouse.
